Community Living-North Frontenac

Transitional Aged Youth (TAY)

TAY is a program designed for youths who are transitioning from the care of Family and Children Services to Adult Developmental Services.


To be eligible for the TAY program people must be between the age of 16-18,  in the care of a provincial child protection service, and have been diagnosed with having a developmental disability or Autism.


Many of these youth do not have family members that can support them to grow and learn the skills  that average people take for granted.  This process to support TAY will enable them to learn life skills that will lead to getting a job, go to post-secondary school, get an apartment.

Key Features

The individual must be between 16-18 years of age

Assist individuals in learning life skills leading to employment, post-secondary education and independent living.

How We Can Help

  • Support individuals with intellectual disabilities achieve their personal goals
  • Maintain roles and relationships
  • Community participation
  • Advocating for the person’s rights
  • Support to look for / maintain a job
  • Enroll in school
  • Transportation (to appointments, to school, outings with family/ friends)
  • Support managing medication
  • Support with ODSP paperwork

As Direct Support professionals we want the best for every person who comes into our service. We thrive to help the individual live the life that they want, without feeling like it isn’t possible.  In anyway we can support an individual to achieve this we will work as hard as we can on their behalf for it to happen.
